NEC 250.110 ground violation: Skipping the ground wire on a fan is a code violation and a shock risk if the motor ever shorts. Twist the bare copper under that green screw, tighten it snug, and don't tape over it.

Step 5 — Blades, power, balance (30 minutes)

Motor noise — hum, grinding, or click — is almost never the cause in fans under 5 years old. Fix the four above and you still hear noise, the bearing is shot and the fan is done.
